There must be at least one <br /> emergency coordinator on site or on call to coordinate emergency response measures, and the following <br /> information must be posted by a phone:the name and phone number of the emergency coordinator; location of fire <br /> extinguishers, spill control equipment, and if present, fire alarm; and the phone number of the fire department, <br /> unless the facility has a direct alarm. Immediately appoint an emergency coordinator and post the required <br /> information by a phone. A form is provided that can be used for this purpose. <br /> This was corrected on site. <br /> This is a Class II violation. <br /> 110 CCR 66262.40(a) Failed to keep signed copy of manifests from the designated facility for three years. <br /> Copies of uniform manifests for the disposal of hazardous waste for 2016 and 2015 were not found on site. <br /> Hazardous waste generators shall retain copies of all manifests signed off by the disposal facility and all receipts <br /> used in a consolidated manifesting procedure on site for three years and have them readily available for review. <br /> Immediately locate a copy of all manifests and receipts for the last three years, maintain them on site, and submit <br /> copies to the EHD. <br /> This is a repeat violation, Class II. <br /> 117 HSC 25185(c)(3) Failed to submit a written response within 30 days of receiving an inspection report. <br /> An inspection was last done on 06/24/2013 and an inspection report was issued identifying information to be <br /> submitted to bring this site into compliance. This information was required to be submitted by 7/24/2013. An <br /> operator that receives an inspection report shall have 30 days to submit a written response that includes a <br /> statement documenting corrective actions taken or proposing corrective actions which will be taken. Ensure that a <br /> written response documenting corrective actions taken or proposed is submitted within 30 days of receiving an <br /> inspection report. <br /> This is a Class II violation. <br /> 601 CCR 66262.34(d) Stored hazardous waste onsite greater than 180 days. <br /> Facilities who generate less than 1000 kg of hazardous waste per month and do not exceed 6000 kg of waste <br /> stored on site at any time may store waste on site up to 180 days. In the waste storage area, a black 55 gallon drum <br /> with a hazardous waste label marked with contents of gas and water on the lid is lacking an accumulation date. <br /> Kornai Bhardwaj,assistant manager, confirmed with inspector on site that the drum was not empty and something <br /> was inside. Since it cannot be determined how long the hazardous waste has been on site, immediately contact a <br /> licensed hazardous waste hauler to dispose of this waste under manifest.
