Reporting a Spill Caused by Vehicle Accident <br /> If the spill was caused by an accident involving vehicles, first, contain the spill, then, call the <br /> police and collect information from the responsible parties. <br /> 1. Call the police if the spill was caused by an accident involving vehicles. The accident <br /> will need to have a police report so Flying J can make an insurance claim against the <br /> responsible party. <br /> 2. Next, collect information from the responsible parties. The police may choose not to <br /> get involved with an accident because it happened on private property. If this is the <br /> case, you need to collect personal information from the responsible parties for <br /> insurance claim purposes. Ask the responsible party for their drivers license and <br /> insurance information. Make photocopies of these documents. If possible, write <br /> down the vehicle license number and truck number. Take pictures of the spill and the <br /> vehicle that caused the spill. (If your plaza does not have a digital camera, use one <br /> from c-store stock, or use a cell phone camera.) <br /> 3. Finally, fill out a property damage report. This form is located on the Flying J <br /> Intranet in the Library. It's in the Safety Manual, in "Section 6: Injury-Accident <br /> Reporting". Print the form and fill it out, send a copy to your district manager and to <br /> the corporate office, and, save a copy in Section 18 of your FOCUS binder. <br /> 10 <br />
