Current Financial responsibility documents have not been submitted to the EHD. The certificate of financial responsibility was last signed on 5/17/17. Current financial responsibility documents are required to be submitted annually. Immediately log into the California Environmental Reporting System(CERS)at http://cers.calepa.ca.gov/, and upload the required documents. The operator, Kevin Hur, uploaded the current financial responsibility to CERS while the inspector was on site. The CERS submittal has not been reviewed by the inspector and the violation will be closed once the submittal has been reviewed and accepted. This is a repeat violation, Class II. 109 CCR 2637(a-d), 2712 Failed to test secondary containment, or failed to maintain test records for 36 months. UDC 3/4, 7/8 and 9/10 were repaired and retested under SR0077415 on 5/16/17 and a six month testing was due by November 2017.The test was completed on 1/23/18 two months past due. Secondary containment testing is required six months after installation and at least every 36 months after. This was corrected on site. This is a repeat violation, Class II. 110 CCR 2637(e), 2637(f) Failed to submit"Secondary Containment Testing Report Form"within 30 days after the test. Secondary containment testing was performed on 5/16/2017 and 1/23/18. A test report has not been submitted to the EHD or the California Environmental Reporting System (CERS). A copy of the test report must be submitted within 30 days of the test. Immediately provide this report to the EHD. This is a repeat violation, Class II. 113 CCR 2638(c), 2638(d) Failure to submit"Monitoring System Certification Form"within 30 days of completion of the test. Annual monitoring system certification, and leak detector testing was performed on 12/28/17 and a test report was submitted on 9/11/18, over 7 months late. A copy of the test report must be submitted within 30 days of the tests. Take all necessary precautions to ensure testing and submittal of test reports are performed in a timely manner. This is a Class II violation. 121 CCR 2712(f) Failed to submit a written response within 30 days of receiving an inspection report. An inspection was last done on 12/28/17 and an inspection report was issued identifying information to be submitted to bring this site into compliance. This information was required to be submitted by 1/27/18. This information was received on 4/2/18, over two months late resulting in a non-compliant status for this facility. An operator that receives an inspection report shall have 30 days to submit a written response that includes a statement documenting corrective actions taken or proposing corrective actions which will be taken. Ensure that a written response documenting corrective actions taken or proposed is submitted within 30 days of receiving an inspection report.
