Any material that meets <br /> or exceeds the reportable quantity shall be reported in the facility's business plan. Immediately log into the <br /> California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, add the materials to the hazardous <br /> material inventory, and submit to the EHD for approval. <br /> The Hazardous Materials Inventory form for the 255 gallons of sodium hydroxide, solution has wrongly been <br /> identified as a solid (see common name and the physical state). This information must be current and submitted as <br /> part of the business plan. Immediately log into the California Environmental Reporting System (CERS) at <br /> http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. <br /> This is a minor violation. <br /> 6 HSC 25505(a)(2) Failed to complete and/or submit a site map with all required content. <br /> The evacuation map is not legible and the site maps do not address internal roads, adjacent streets, storm and <br /> sewer drains, access and exit points (facility and structures), evacuation staging areas, hazardous material handling <br /> and storage areas (citric acid and boiler treatment chemical, and emergency response equipment(fire extinguishers <br /> and spill control). A site map shall contain a north orientation, loading areas, internal roads, adjacent streets, storm <br /> and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous material <br /> handling and storage areas, and emergency response equipment. If a site map element is not applicable for your <br /> facility then list it on the map and label as "NA". Immediately log into the California Environmental Reporting System <br /> (CERS) at http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br /> This is a minor violation. <br /> 9 HSC 25508(a)(1) Failed to annually review and certify that the business plan is complete, accurate, and up to date. <br /> The business plan information has not been reviewed and resubmitted in the California Environmental Reporting <br /> System (CERS) annually since 2014. The hazardous materials inventory shall be submitted by January 15 of each <br /> calendar year and may be submitted beginning November 1 of the previous year. Immediately log into the CERS at <br /> http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. <br /> This was corrected on site. <br /> This is a Class II violation. <br /> 11 HSC 25505(a)(4) Failed to provide or document initial and/or refresher training to appropriate personnel. <br /> Employee training records were not available at the time of inspection. The business plan shall include provisions <br /> for ensuring that appropriate personnel receive initial and annual refresher training. This training shall be <br /> documented electronically or by hard copy and shall be made available for a minimum of three years.
