The following is an itemized list of hazardous materials violations that have not been addressed for PEARL CROP INC as of February21, 2019. Open violations from January 03, 2019 inspection Violation #5 - Failed to complete and/or submit hazardous materials inventory for all reportable materials on site. Twelve 30.961 kg. cylinders of phosphine fumigant gas, one 55 gallon drum of Alpet sanitizer(flammable), one 55 gallon drum of Hydro 1000 (potassium hydroxide), 627 cubic feet of compressed oxygen, 387 cubic feet of acetylene, one est. 400 pound container of liquid nitrogen (actual size not available), and 2,004 cubic feet of compressed gas n.o.s, UN 1956 were found on site that have not been reported. Any material that meets or exceeds the reportable quantity shall be reported in the facility's business plan. Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, add the materials to the hazardous material inventory, and submit to the EHD for approval. Violation #6 - Failed to complete and/or submit a site map with all required content. The map does not show loading areas, access and exit points, and emergency response equipment(fire extinguishers). A site map shall contain a north orientation, loading areas, internal roads, adjacent streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous material handling and storage areas, and emergency response equipment. If a site map element is not applicable for your facility then list it on the map and label as"NA". Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. Violation#10 - Failed to submit revisions to business plan within 30 days of substantial change in operations. During the inspection, twelve 30.961 kg. cylinders of phosphine fumigant gas, one 55 gallon drum of Alpet sanitizer (flammable), one 55 gallon drum of Hydro 1000 (potassium hydroxide), 627 cubic feet of compressed oxygen, 387 cubic feet of acetylene, and 2,004 cubic feet of compressed gas n.o.s, UN1956 were found on site and the updated information has not been submitted. Within 30 days of one the following events, business shall electronically update the information submitted to the California Environmental Reporting System (CERS): -A 100 percent or more increase in the quantity of a previously disclosed material. -Any handling of a previously undisclosed hazardous materials -A change of business address, business ownership, or business name. Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. Violation#12 - Failed to have the business plan readily available to facility or EHD personnel. Facility could not provide the Emergency Response Plan and the Chemical inventory upon request. The emergency response plans and procedures, the inventory of information required by this article, and the site map required by this section shall be readily available to personnel of the business or the unified program facility with responsibilities for emergency response or training pursuant to this section.
