The following is an itemized list of hazardous materials violations that have not been <br /> addressed for PEARL CROP INC as of February21, 2019. <br /> Open violations from January 03, 2019 inspection <br /> Violation #5 - Failed to complete and/or submit hazardous materials inventory for all reportable materials on <br /> site. <br /> Twelve 30.961 kg. cylinders of phosphine fumigant gas, one 55 gallon drum of Alpet sanitizer(flammable), one 55 <br /> gallon drum of Hydro 1000 (potassium hydroxide), 627 cubic feet of compressed oxygen, 387 cubic feet of <br /> acetylene, one est. 400 pound container of liquid nitrogen (actual size not available), and 2,004 cubic feet of <br /> compressed gas n.o.s, UN 1956 were found on site that have not been reported. Any material that meets or <br /> exceeds the reportable quantity shall be reported in the facility's business plan. Immediately log into the California <br /> Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, add the materials to the hazardous material <br /> inventory, and submit to the EHD for approval. <br /> Violation #6 - Failed to complete and/or submit a site map with all required content. <br /> The map does not show loading areas, access and exit points, and emergency response equipment(fire <br /> extinguishers). A site map shall contain a north orientation, loading areas, internal roads, adjacent streets, storm <br /> and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous material <br /> handling and storage areas, and emergency response equipment. If a site map element is not applicable for your <br /> facility then list it on the map and label as"NA". Immediately log into the California Environmental Reporting System <br /> (CERS) at http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br /> Violation#10 - Failed to submit revisions to business plan within 30 days of substantial change in <br /> operations. <br /> During the inspection, twelve 30.961 kg. cylinders of phosphine fumigant gas, one 55 gallon drum of Alpet sanitizer <br /> (flammable), one 55 gallon drum of Hydro 1000 (potassium hydroxide), 627 cubic feet of compressed oxygen, 387 <br /> cubic feet of acetylene, and 2,004 cubic feet of compressed gas n.o.s, UN1956 were found on site and the updated <br /> information has not been submitted. Within 30 days of one the following events, business shall electronically update <br /> the information submitted to the California Environmental Reporting System (CERS): <br /> -A 100 percent or more increase in the quantity of a previously disclosed material. <br /> -Any handling of a previously undisclosed hazardous materials <br /> -A change of business address, business ownership, or business name. <br /> Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, enter the <br /> correct or updated information, and submit to the EHD for approval. <br /> Violation#12 - Failed to have the business plan readily available to facility or EHD personnel. <br /> Facility could not provide the Emergency Response Plan and the Chemical inventory upon request. The emergency <br /> response plans and procedures, the inventory of information required by this article, and the site map required by <br /> this section shall be readily available to personnel of the business or the unified program facility with responsibilities <br /> for emergency response or training pursuant to this section.
