This information was required to be submitted by 2/24/18. This information has <br /> not been received resulting in a non-compliant status for this facility. An operator that receives an inspection report <br /> shall have 30 days to submit a written response that includes a statement documenting corrective actions taken or <br /> proposing corrective actions which will be taken. Ensure that a written response documenting corrective actions <br /> taken or proposed is submitted within 30 days of receiving an inspection report. <br /> Open violations from the 1/25/16 inspection report are: <br /> -Failed to keep signed copy of manifests from the designated facility for three years. <br /> -Failed to comply with uniform hazardous waste manifest exception requirements. <br /> This is a Class II violation. <br /> 601 CCR 66262.34(d) Stored hazardous waste onsite greater than 180 days. <br /> A blue 55 gallon steel drum located by Dispenser 5/6 did not have a starting accumulation date and the facility's last <br /> disposal records found on site show to be on 1/31/18.When the employee was asked when the accumulation start <br /> date was, he was not able to provide an answer. Facilities who generate less than 1000 kg of hazardous waste per <br /> month and do not exceed 6000 kg of waste stored on site at any time may store waste on site up to 180 days. <br /> Immediately contact a licensed hazardous waste hauler to dispose of this waste under manifest and submit a copy <br /> of the manifest to the EHD. <br /> This is a Class II violation. <br /> 605 CCR 66262.34(f) Failed to completely label containers or portable tanks of hazardous waste. <br /> A 55 gallon blue steel drum located next to dispenser 5/6 did not have the proper accumulation start date, nor <br /> hazardous properties labeled. All hazardous waste containers shall be marked with the following information: <br /> -the words"Hazardous Waste" <br /> -name and address of generator <br /> -hazardous properties <br /> -physical state <br /> -composition(contents) <br /> -accumulation start date <br /> Immediately label these containers and ensure that all hazardous waste containers are marked with all the required <br /> information. <br /> This is a minor violation. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by February 6th, 2019 . <br /> Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate($152).
