SANJ 0 A U I Environmental Health Department <br /> 0U NT <br /> Hazardous Materials Program Inspection Report <br /> Facility Name: Facility Address: TDate: <br /> A G SPANOS AVIATION DEPT* 4800 S AIRPORT WAY, STOCKTON January 10, 2019 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 5 HSC 25505(a)(1) Failed to complete and/or submit hazardous materials inventory for all reportable materials on site. <br /> The Hazardous Materials Inventory forms for jet fuel and waste oil were not complete. The storage pressure for the <br /> jet fuel and the annual waste amount for the waste oil have not been identified. This information must be current and <br /> submitted as part of the business plan. Immediately log into the California Environmental Reporting System (CERS) <br /> at http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. <br /> This is a minor violation. <br /> 6 HSC 25505(a)(2) Failed to complete and/or submit a site map with all required content. <br /> The map does not show loading areas, adjacent streets, and a legend to identify the code used to show the waste oil <br /> storage (5). A site map shall contain a north orientation, loading areas, internal roads, adjacent streets, storm and <br /> sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous material handling <br /> and storage areas, and emergency response equipment. If a site map element is not applicable for your facility then <br /> list it on the map and label as "NA". Immediately log into the California Environmental Reporting System (CERS) at <br /> http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br /> This is a minor violation. <br /> 11 HSC 25505(a)(4) Failed to provide or document initial and/or refresher training to appropriate personnel. <br /> Employee training records were not available at the time of inspection. The business plan shall include provisions for <br /> ensuring that appropriate personnel receive initial and annual refresher training. This training shall be documented <br /> electronically or by hard copy and shall be made available for a minimum of three years. Immediately provide <br /> employee training for appropriate personnel and submit a copy of the training records to the EHD. <br /> This is a Class II violation. <br /> 12 HSC 25505(c) Failed to have the business plan readily available to facility or EHD personnel. <br /> Facility could not provide the Emergency Response Plan and the Chemical inventory upon request. The emergency <br /> response plans and procedures, the inventory of information required by this article, and the site map required by <br /> this section shall be readily available to personnel of the business or the unified program facility with responsibilities <br /> for emergency response or training pursuant to this section.
