Subject: RE: Tesla Treatment Facility update December 2018 <br /> Hi Brendan, Garrett, <br /> Below is a summary on the trench clean up and interception plan. <br /> On 12/20 a lamp break occurred. The sleeve was intact and the reactor was offline. There was no <br /> discharge of mercury into the water system. The crew onsite followed the mercury response SOP <br /> and removed and replaced the lamp and sleeve. The broken lamp was bagged and tagged and <br /> placed in our hazardous waste storage area. <br /> On 12/27 another lamp break occurred. Water and mercury were discharged into the trench. Due to <br /> reactor panels being removed for repair related to last week's break, the water pressure from the <br /> break on reactor 2120 damaged some internal components on 2110. Operations staff did not call <br /> OES, because there was no discharge to the environment. Voice messages were left with Regional <br /> Board and San Joaquin County OEH representatives that we have been working with on this <br /> problem. An incident report will be filed with Cal OES and San Joaquin County CUPA. The majority of <br /> the water was pumped to the baker tanks on 12/27 via the emergency system put in place last <br /> month. SFPUC and DPW used the same contractor that we used to clean out the trench the first <br /> time, CES, to clean out the trench again on 1/10/2019. All wash water was pumped to the baker <br /> tanks, and will be hauled out in a vacuum truck by CES on Monday, 1/14/2019. A representative <br /> from AEW, Randall Young, verified the trench was clean using an in-situ mercury vapor analyzer. <br /> The scheduled shutdown of the plant for routine maintenance of the 144 inch pipes occurred on <br /> 1/8/2019, and all flows have ceased at Tesla Portal.The site has undergone LOTO for valves and the <br /> UV reactors. The emergency system remains in place should another lamp break occur, but as all the <br /> reactors are currently offline, this shouldn't be a problem. <br /> Calgon inspected the lamps after the breaks, and encountered a third break that apparently failed <br /> during the cool down prior to inspection.The sleeve did not break so no mercury was released <br /> (similar to the break on 12/20). In total 12 out of 108 lamps were found with deformations and <br /> bulging most likely caused by acoustic resonance. Five (5) reactors had no issues with the lamps. <br /> The broken lamps were shipped to Calgon, and given the nature of the deformities they are focusing <br /> on acoustic resonance as the root cause.
