Date: Aug 28, 2017 <br /> Business Name: CHUCK E CHEESE'S Record ID#: C00044067 <br /> Site Address: 6436 PACIFIC AVE Program Element: 1600 <br /> STOCKTON, CA 95207 FOOD PROGRAM <br /> NATURE OF WASTE DISCHARGE/WASTE RELEASE: <br /> COMPLAINANT ALLEGES THEY OBSERVED A RAT DECOMPOSING IN THE FACILITY. <br /> OBSERVATIONS: <br /> Spoke to store manager who was aware of this complaint. He stated that when customer was given the bill for their large <br /> party, and at that moment they stated that their kids found a dead mouse inside the climbing tubes(inside the yellow car). <br /> The manager wanted to see proof, but customer was not able to show them the picture that they stated that they had. <br /> Manager sent a small employee thru the tubes and he/she found a dirty piece of parchment paper with sauce in it. Manager <br /> also stated that FOX 40 news came to site. <br /> Observations:There is no evidence of rodents at time of inspection. Last 2 pest control service was on 07/21/17 and <br /> 08/27/17 . On report it stated that there in no rodent activity. <br /> Demonstration of Knowledge <br /> ° Demonstration of Knowledge <br /> All food employees shall have adequate knowledge of and be trained in food safety as it relates to their assigned duties. <br /> (113947)Food facilities that prepare, handle or serve non-prepackaged potentially hazardous food, shall have an employee <br /> who has passed an approved food safety certification examination. (113947-113947.1)Any food handler hired after June <br /> 1, 2011 shall obtain a Food Handler Card within 30 days(113948). <br /> Employee Health and Hygiene <br /> Employee Communicable Disease Controls <br /> Employees with a communicable disease shall be excluded from the food facility/preparation of food. Gloves shall be worn <br /> if an employee has cuts, wounds, and rashes. No employee shall commit any act that may contaminate or adulterate food, <br /> food contact surface, or utensils. (113949.5). The permit holder shall require food employees to report incidents of illness or <br /> injury and comply with all applicable restrictions. (113949.2, 113950,113950.5, 113973(a)) <br /> Employees with Discharge from Eyes, Nose, Mouth <br /> Employees experiencing sneezing, coughing, or runny nose shall not work with exposed food, clean equipment, utensils or <br /> linens. (113974) <br /> ° Employee Eating, Tasting, Drinking or Tobacco Use <br /> No employees shall eat, drink, or smoke in any work area. (113977) <br /> Preventing Contamination by Hands <br /> ° Hands Clean/Properly Washed/Proper Glove Use <br /> Employees are required to wash their hands:before beginning work before handling food/equipment/utensils as often as <br /> necessary, during food preparation, to remove soil and contamination when switching from working with raw to ready to eat <br /> foods, after touching body parts after using toilet room or any time when contamination may occur.
