Violations that are classified as"MAJOR"pose an immediate threat to public health <br /> and have the potential to cause foodborne illness.All major violations must be corrected immediately.Non-compliance may warrant immediate closure of <br /> the food facility. <br /> #23 Rodents, Insects or Animals Inside Facility <br /> OBSERVATIONS:Rodent droppings were observed in the following locations: on top of the dish machine, on the floor <br /> underneath the dish machine, on the Hatco water heater booster, on top of the ovens, on the floor under shelving in the dry <br /> goods storage rooms, in the small room by the walk-in cooler(paper goods storage), and under the steam tables at the <br /> serving line. <br /> Observed gnawed on condiment packets under the steam tables at the serving line. <br /> Observed rodent paw prints under the steam tables at the serving line. <br /> Take immediate action to remove all rodent droppings throughout the kitchen areas, storage areas and serving areas. Clean <br /> and sanitize the areas. <br /> Monitor facility daily for any new activity. <br /> Per Food Service Supervisor, Clark Pest Control services the facility 2x/month. <br /> Submit copies of pest control service invoices to Kadeanne Linhares(klinhares@sjcehd.com)for late November and early <br /> December. <br /> CALCODE DESCRIPTION:Each food facility shall be kept free of vermin:rodents(rats,mice), cockroaches,flies.(114259.1, 114259.4, <br /> 114259.5) <br /> #44 Premises: Clean/Litter Free; Vermin-Proof <br /> OBSERVATIONS:The two rear sets of doors of the kitchen are being left open. Maintain doors closed at all times. <br /> The right side set of doors have a substantial gap in between the doors and at the bottom of the doors.Take action to <br /> eliminate the gap.
