An inspection was last done on January 29, 2018 and an inspection report was issued identifying information to be submitted to bring this site into compliance. During the January 29, 2018 inspection, water was found in the diesel, 87, and 91 submersible turbine pump (STP) sumps, 87 and diesel fill sumps, vent box sump, and under dispenser containment (UDC) sump in dispenser#3/4. The received return to compliance documentation did not adequately address this violation. This information was required to be submitted by March 1, 2018. The designated operator failed to document all the alarms from the attached alarm history on the April 26, 2018 designated operator monthly inspection report and failed to check that they were responded to appropriately. The missing alarm includes: Q3 alarm which occurred on April 6, 2018. During the monthly inspection, the designated operator shall review the alarm history for the previous month, check that each alarm was documented and responded to appropriately, and attach a copy of the alarm history with documentation taken in response to any alarms to the monthly report. Ensure that designated operators performing monthly inspections at this facility are including all of the required information on the reports.
