Environmental Health Department <br /> SAN JOAQUIN <br /> jUNTY— <br /> Complaint Record Daily Time and Activity Summary from 8/1/2012 to <br /> 8/112017 for COIGJWK6C located at <br /> Activijy Date Recorded by <br /> 06/16/2017 Advised by Kasey to generate a complaint and follow through with Michelle H, PE 1921 LOPEZ <br /> 30 min generated complaint; SC 004 <br /> 08/09/2017 1 arrived at the shop to verify the report of drums of hazardous materials are being PE 1921 LOPEZ <br /> 96 min stored in this facility; the roll-up doors of the shop were open and the shop was full SC 004 <br /> of vehicles; I asked a worker if the manager or owner were present; I was pointed <br /> to a man who identified himself as Mario Padilla; I identified myself to Mr. Padilla <br /> and advised I was at the site to verify the presence of hazardous materials; Mr. <br /> Padilla showed me around the shop, advised he will be running the shop for his <br /> aunt, that the shop is not yet open for business and they are applying for an EPA <br /> ID number; I did not see any drums or any reportable amount of hazardous <br /> material in the shop; I advised Mr. Padilla I was attempting to verify the report of <br /> drums being stored inside this shop and asked if he had seen or knew of any <br /> drums being stored there; he said he did, was not sure how many but he thinks he <br /> saw at least 10 drums; he said he had paid for the site, but property management <br /> would not allow him to move in until sometime in mid-July atwhich time the drums <br /> were gone; I asked Mr. Padilla if he know what happened to the drums and he <br /> said a man named Albert had the drums moved; I asked if he could call albert or <br /> provide me with Albert's contact information; Mr. Padilla was in the process of <br /> calling Albert and writing down his phone number(209) 992-1102, when Albert <br /> Laps drives up; I identified myself to, Mr. Lepe, advised I was there to see the <br /> drums and asked him what happed to the drums;Albert said they were moved to <br /> another area of the property, I asked him if he could show me the drums and he <br /> said to follow him and took me to a shop at the west end of the business park; <br /> walking up to the shop I could see many drums, stacked up inside the west side of <br /> the shop; I asked if this was the maintenance shop for the business park; Mr. Lepe <br /> said no, but they are using it because the tenant, Costa, recently passed, and <br /> they inherited the drums that were left; Mr. Lepe said the City of Stockton, and he <br /> mentioned Rico, inspected the shop last week and guessed that was why I was <br /> there; I told Mr. Lepe I was not aware the Stockton had been there but, I advised <br /> Mr. Lepe I too needed to take photos and complete an inspection; I requested and <br /> received consent from Mr. Lepe to conduct an inspection and take photos; I took <br /> photos and documented violations 2, 3, 4, 5, 6, 7, 8, 9, 11 and 12 on a check list. <br /> 08/09/2017 Conducted site inspection; documented violations 2, 3, 4, 5, 6, 7, 8, 9, 11 and 12 PE 1921 LOPEZ <br /> 30 min on a check list; returned to office and advised Garrett of the event; Garrett advised SC 007 <br /> the drum/totes are an ongoing issue and are related to 000042920; <br /> 08/10/2017 Completed 004 and 007 narratives for yesterdays dailies; generated pink and PE 1921 LOPEZ <br /> 300 min green forms forms; discussed with Garrett the amount of chemicals to complete SC 007 <br /> the pink be set at the low side of 10 chemicals, completed an email to Muni and <br /> Michelle H regarding containers as Garrett instructed; added text to photos and <br /> parcel view overhead; generated and printed a LOG 2 report; scanned the report <br /> documents to server; forwarded the report to Garrett for review; Facility ID <br /> FA0024191 and PR0542119 assigned. <br /> Page 1 of 1 <br />
