Telephone: (209) 468=3420 Fax: (209) 464-0138 Web: www.sjgay.org/ehd <br /> Q�rFp'A� <br /> AUTHORITY <br /> The San Joaquin County Environmental Health Department (EHD), as authorized by the Housing and Nuisance <br /> Abatement Code of San Joaquin County, has inspected the aforementioned premise. This premise has been <br /> found to be in violation of Title 8, Division 5, of the San Joaquin County Ordinance Code; the California Health <br /> and Safety Code, Section 17920.3; and the California Code of Regulations, Title 25, Division 1, Subchapter 1. <br /> COMPLIANCE <br /> The listed violations identified on the reverse side of this form must be corrected within 30 days of notification of <br /> the violations. If the property is encumbered by a recorded mortgage or deed of trust, an additional 15 days will j <br /> be granted for the mortgagee or beneficiary to comply with the requirements of this notice. The cost to the <br /> mortgagee or beneficiary shall be added to and become part of the lien secured by said mortgage or deed of trust <br /> (California Code of Regulation, Title 25, Division 1, Subchapter 1, Section 54). <br /> Failure to correct the violation within the time constraints identified in this notice will result in further legal action. <br /> ENFORCEMENT COST ' <br /> The property owner of record shall be responsible for all County costs associated with the enforcement of this <br /> NOTICE. <br /> The owner of record may also be liable for tenant relocation costs involving any occupied structures that are a <br /> I <br /> threat to public health and safety. <br /> i <br /> PERMIT - <br /> Prior to commencing work, you must secure the required permits from the San Joaquin County Building Division <br /> located at 1810 East Hazelton, Stockton, CA. The Building Division's telephone number is (209) 468-3121. All <br /> work associated with the septic system or any water well supply must be conducted under permit and inspection <br /> by the EHD located at 600 East Main Street, Stockton, CA. The EHD telephone number is (209) 468-3420. All <br /> required permits are subject to fees. <br /> REHABILITATION ASSISTANCE <br /> Questions regarding your participation in the Neighborhood Preservation Program for rehabilitation assistance <br /> can be answered through the San Joaquin County Preservation Division located at 1810 East Hazelton, Stockton, <br /> CA. The Preservation Division's telephone number is (209) 468-3021. <br /> STATE OF CALIFORNIA REVENUE AND TAXATION CODE <br /> California Revenue and Taxation Code Sections 17299 and 24436.5 require that the County report all <br /> substandard rental units that do not comply within six months of this NOTICE. If compliance is not obtained, this <br /> office will report such noncompliance to the State Franchise Tax Board which will disallow the State Income Tax <br /> deduction of interest, depreciation, or taxes for said dwelling unit(s). A NOTICE OF VIOLATION will also be <br /> recorded with the San Joaquin County Recorder's Office. <br /> APPEALS <br /> The recorded owner and any other parties holding recorded legal interest will be notified of this action and may <br /> appeal this action or the indicated violations to the San Joaquin County Board of Housing Appeals. All appeals <br /> shall be filed within 30 days of the date of this NOTICE. <br /> Appeals may be delivered or mailed to the EHD at 600 East Main Street, Stockton, CA 95202. The EHD <br /> telephone number:is (209) 468-3420. All appeals to the San Joaquin County Board of Housing Appeals are <br /> subjectto a.fiiing fee. <br /> HOUSING ASSISTANCE <br /> Information regarding housing assistance may be obtained through the Housing Authority of the County of San <br /> Joaquin at 448 South Center Street, Stockton, CA. The Housing Authority telephone number is 460-5088. <br /> COUNSELING, MEDIATION ASSISTANCE <br /> Information regarding tenant or landlord rights and mediation or counseling services may be obtained through <br /> San Joaquin Fair Housing at 247 East Miner Avenue, Suite A, Stockton, CA. The Fair Housing telephone number <br /> is (209) 451-3471. <br /> EMERGENCY ABATEMENT <br /> There is no appeal for emergency abatement.
