Mass causality incident(MCI) <br /> Bayside Landing <br /> 3318 Brookside Rd., Stkn. <br /> July 29,2001 <br /> 9:30 a.m. I received pager notification and returned call to 468-6268. I spoke Eunice at S.J.Co. <br /> Hospital Emergence Room. She requested the Health Officer and I referred her to Kathy Tully Public <br /> Health nurse. <br /> 9:40 a.m. I received a call from Tully and she briefed me on MCI and referred me to Mike Lame at <br /> S.J.Hosptial Emergency. <br /> 9:45a.m. I phoned Mike Lame and discussed Stockton Fire Dept.'s request for food inspection and <br /> gave me Robert Mankin as a contact. <br /> 9:50a.m. I phoned Mankin and spoke with him regarding his request for food inspection. I told <br /> him I would call Doug Wilson,my supervisor,for guidance. Spoke with Barbara at Bayside regarding <br /> situation and Permits for facility. She stated the facility was permitted by State of California Dept.of <br /> Social Services and not S.J.Co.PHS/EHD. <br /> 10;00a.m. I phone Wilson and he referred me to Al Olsen,EHD Progam Manager,for direction <br /> regarding Fire's request for response and inspection. <br /> 10;l0a.rr I received another phone call from Lame requesting EHD response to incident. <br /> 10:15a.rr I spoke with Tully regarding incident and Lame's request regarding MCI and her <br /> assistance in evaluating cause of the residence illnesses. <br /> 10:20a.m. I phone Olsen and discussed Fire's request for EHD's response and Tully's assistance. <br /> Olsen stated EHD has authority to inspect,even though Bayside was not an EHD permitted facility. <br /> 10:40a.m. I received a phone call from Daryl Cramphorn,EMS Director,requesting EHD response <br /> to incident. I told him I was on my way to Bayside and would give him a updated after my inspection. <br /> 10:45a.m. I phoned Bayside to speak with Manikin to tell him I was responding and would <br /> complete an inspection,but he was unavailable,so I left a message with Barbara to tell Fire I was in route. <br /> 10:45a.m. I phoned Wilson with an update and left for Bayside. <br /> 10:55a.m. I received a page from Kathy Tully and returned her call and she stated she would met <br /> me at Bayside for assistance in investigating the cause of the illnesses. <br /> 11:OOa.m. I phoned Wilson and told him Tully was also responding to S.J.Hospital and Fire's <br /> request in investigation to cause of MCI. <br /> 11:32a.m. I arrived at Bayside and began my inspection, see report. <br /> 2:l Op.m. After the inspection was completed and review with Tully and Lichtenhan, I left the <br /> facility. <br /> 2:15p.m. I phoned Daryl Cramphorn and gave him and updated:no apparent food violation <br /> evidenced and 4 samples(one water and 3 food)were collected for possibly analyses at a later date. <br />
