SANJ O A Q U I N Environmental Health Department <br /> COUNTY-- <br /> Complaint Record Daily Time and Activity Summary from 3/1/2015 to <br /> 6/2/2034 for COKPHKOBO located at <br /> Byron Rd X Mountain House Parkway <br /> Activity Date Recorded by <br /> 02/21/2018 Responded to Byron Rd. at Henderson Rd. to assess and cleanup a sodium PE 2546 LOPEZ <br /> 180 min hypochlorite spill; eventually the responsible party (RP) arrived and preformed SC 004 <br /> cleanup; provided RP with one 5 gallon plastic pail for waste containerization; <br /> released by Garrett. <br /> 02/21/2018 Notification of Hazardous Waste Discharge. PE 2546 BACKUS <br /> 30 min SC 203 <br /> 02/21/2018 5-8 am. Received call at 5:30 am from Muniappa Naidu (SJCEHD)via SJGH PE 2546 BACKUS <br /> 180 min operator asking me to respond to a chlorine spill on Byron Rd. At this time there is SC 974 <br /> no responsible party. I called CHP dispatch at 943-8600 and told them my ETA <br /> will be approximately one hour. Traffic around the site caused a delay and I <br /> arrived on site at approximately 7:15 am. Byron Road was closed to traffic. <br /> Liquid with a bleach smell had spilled onto the east bound lane of Byron Rd at <br /> Mountain House Parkway. An area of approximately 20 feet by 7 feet contained a <br /> puddle of the liquid. An area of approximately 250 feet by 40 feet was moist with <br /> the liquid but was not a puddle. I discussed the situation with CHP Srgt Lecce and <br /> J Gish of Mountain House Fire. I called Muniappa and he dispatched Robert <br /> Lopez to conduct the cleanup. <br /> 02/21/2018 8 am. At 8:30 am Srgt Lecce informed me that a responsible party had been PE 2546 BACKUS <br /> 240 min found and is responding to the site. The spilled chemical is 12%sodium SC 544 <br /> hypochlorite aqueous solution coming from a tanker truck. The truck driver had <br /> spilled the chemical on a car and the car driver informed law enforcement. Robert <br /> Lopez arrived on site at approximately 8:45 am. I asked him to standby until the <br /> cleanup was in motion by the responsible party. <br /> The responsible parties Richard Doss and Dave Johnson of Hasa Pool arrived on <br /> site at approximately 9 am and organized cleanup of the puddle of liquid. I issued <br /> a Notice to Abate to Dave Johnson. A Hasa crew used diatomaceous earth to <br /> cleanup the spill and placed the waste in a five gallon bucket with a hazardous <br /> waste label on it. Richard Doss attempted to contact a hazardous waste <br /> transporter to dispose of the waste. I suggested that they could take the waste to <br /> the SJC Hazardous Waste (HHW) facility. Dave Johnson called the HHW to <br /> arrange a business appointment. I left the site at approximately 10:45 am. <br /> Richard Doss emailed me the HHW receipt for the waste disposal at 2:39 pm. <br /> Finished documentation and abated the complaint. <br /> Page 1 of 1 <br />
