SANJ O A Q U I N Environmental Health Department Complaint Record Daily Time and Activity Summary from 1/9/2018 to 1/9/2019 for COVXOJEPW located at 1878 Industrial Dr., Suite C Activity Date Recorded by 12/31/2018 While attempting to conduct a closure inspection, I observed at least 5 fluorescent PE 2200 LOPEZ 30 min light tubes and an aerosol can inside a blue Republic Services roll off bin in the SC 004 alley behind 1878 Industrial, Suite C.; I called and advised Muni; Muni instructed to generate a complaint. 12/31/2018 Completed complaint and forwarded to Muni. PE 2200 LOPEZ 30 min SC 007 01/07/2019 Received a complaint from Muniappa Naidu. Robert Lopez saw 5 fluorescent light PE 2200 ALANIZ 24 min tubes and an aerosol can inside a blue Republic Serives roll off bin. SC 007 01/07/2019 Answered questions from John A. and Muni regarding the status of the waste; I PE 2200 LOPEZ 30 min advised I was at the site to day to conduct a closure inspection of PR0543828 and SC 007 the roll off bins were no longer present. 01/08/2019 Called Mike Seley (environmental contact for French Oils) at 626-240-1083. No PE 2546 ALANIZ 60 min one answered the phone and his voice mail was full. Called his cell phone at SC 007 415-640-2261. Mike said that if there were fluorescent light tubes in the trash, then they would not have been taken out. Discussed with Garrett Backus. Called him back and told him that I would be writing him a notice to abate. Discussed with Michelle and Muniappa. Called Mike and told him that I would not be writing a notice to abate. He said that the business, French Oils, was going out of business and not moving to antohter location. Wrote the notice to abate. Mike called back saying that his men do not remember seeing any tubes in the trash. He said that he did contract out the roll off bin and that he would take full responsibility. Spoke with Muniappa Naidu who told me to wait on issuing the notice to abate. Called Mike bike and told him that I would be getting back to him. I emailed him the DTSC factsheet on universal waste and discussed how to properly dispose of it. His email is: <mike@seleyco.com> He confirmed that he received the email Spoke with Muniappa Naidu and it was determined that a notice to abate would not be written. Called Mike and informed him that the incident was closed with the county. He said that the business was closing and not moving to another location. 01/09/2019 Pritned out narrative and abated the complaint.
