Annual Monitoring Summary Report <br /> The Discharger shall submit the Annual Monitoring Summary Report to the Board by 15 January <br /> each year. <br /> d. Constituents-of-Concern (COC)5-'Rear Report <br /> The Discharger shall submit reports of the results of groundwater monitoring for the Constituents of <br /> Concern every five years, or more frequently if required_ The initial COC Report is due at the end <br /> of the first full quarter of facility operation. The succeeding groundwater monitoring for COC <br /> Report shall alternate between the Pall and Spring seasons. The COC Report may be combined <br /> with a Detection Monitoring Report or an A-"uaI Summary Report having a Reporting Period that <br /> ends at the same tirne. <br /> C. S-X'ANDARD OBSERVATIONS <br /> Each monitoring report shall include a summary and certification of completion of all Standard <br /> Observations for the surface impoundments, for the pen-meter of the surface impoundments, and for the <br /> receiving waters- The standard observations shal be perforrned on a weekly basis and shall include <br /> those elements as defined in the Standard Provisi ns and Reporting Requirements. <br /> D. MONITORING <br /> If the Discharger, through a detection monitoring.program, or the Board finds that there is measurably <br /> signiEcant evidence of a release from a surface impoundment or waste constituents over the water duality <br /> protection standards at or beyond the Points of Compliance, the Discharger shall notify the Board or <br /> acknowledge the Board's finding in writing within seven days, and shall immediately resarnple for the <br /> constituent(s)oz parameter(s) at the point where the standard was exceeded. Within 90 days, the <br /> Discharger shall submit to the Board the results of the resampling and either: <br /> a, a report demonstrating that the water quality protection standard was not, in fact, exceeded; or <br /> b, an amended Report of Waste Discharge for the establishment of a evaluation monitoring program, <br /> per Section 20425 of"Title 27, which is designed to assess the nature and extent of the release from <br /> the unit and to design a corrective action program meeting the requirements of Section 20430. <br /> If the Discharger, through an evaluation monitoring program,or the Board verifies that water quality <br /> protection standards have been exceeded at or beyond the Points of Compliance, the Discharger shall <br /> notify the Board or acknowledge the Board's finding in writing within seven days. Within 180 days, the <br /> Discharger shall submit to the Board an amended Report of Waste Discharge for the establishment of a <br /> corrective action program,per Section 20420 of Title 27, which is designed to achieve compliance with <br /> the water quality protection standards. <br /> zT 'd zb:VT 00, Zz Inf <br />
