Diamond Foods, Inc. <br /> INCIDENT AND CLOSE-CALL REPORT <br /> Name: Robert Castro_ Date of Incident:-1/26/2016— <br /> Incident Location: South Plant Waste water <br /> Shift: 2nd Time of Incident: 7:00PM <br /> Sequence of Events (Describe what happened before, during and after the incident, and current <br /> status of incident) At 7:OOPM tonight I was called by Bill Clark to report a discharge into <br /> the Mormon Slough from our South Waste Water system. Upon Investigation it was <br /> found that a 4" transfer pipe had failed and had pumped Process waste into our Storm <br /> Water system. Facilities mechanics and the sanitation department started working on <br /> the containment of the flow out to the slough. New Wattles were installed to impede <br /> flow of process water at the drain outside the cleaning plant, as well as the outfall to <br /> Mormon slough. Jorge Lopez and Bill Clark coordinated the Installation fo the storm <br /> drain plug as well as the suction pump coordination to remove the remaining standing <br /> water from the affected area. I contacted the necessary regulatory agencies of the <br /> unauthorized discharge per our Spill prevention control and countermeasure plan <br /> (SPCC) <br /> Immediate Actions Taken <br /> Corrective Actions: Owner: Due Date: Completion Date: <br /> 1.
