SA N J OA Q U I N Environmental Health Department <br /> COUNTY— <br /> Complaint Record Daily Time and Activity Summary from 3/1/2015 to <br /> 6/21/2034 for COMAV01 located at <br /> 801 Diamond St <br /> Activity Date Recorded by <br /> 03/06/2017 9:20am: Called BNSF call center(817-352-2833).Spoke to Dave. He did not have PE 2546 RIVERA <br /> 180 min any updated information and stated James Fanner is overseeing the spill and SC 544 <br /> would have the most recent information. Left a message for James Fanner <br /> (909-267-5167) requesting an update on the spill. <br /> 9:58am: Arrived on site. Met with Train Masters George Vash and Alan Maas. <br /> According to Mr.Vash the track is currently not safe; he requested that we meet <br /> back at the site at 11:30am with Alan Maas to verify that the spill cleanup was <br /> complete. <br /> 10:07am: Left site. <br /> 11:25am: Arrived on site. Met with Allen Maas and Christopher Ortiz, Mechanical <br /> Foreman. They escorted us to the spill area on track 32. According to Mr. Ortiz, <br /> Environmental Logistics was called out and discovered the spill was coming from <br /> the overfill line on the locomotive causing a spray on the track. Environmental <br /> Logistics cleaned the locomotive and removed approximately 2 inches of ballast. <br /> The contaminated ballast was placed in one 55-gallon drum for disposal. No oil <br /> was observed on the ballast at the spill area. Mr. Ortiz stated the disposal record <br /> could be provided by James Farrier. Mr. Maas requested a copy of the Notice to <br /> Abate be emailed to him. <br /> 12:01 pm: Left site. <br /> Received a message from James Farrier that the diesel only sprayed onto the <br /> surface of the ballast in a section approximately 21ft in length. <br /> 2:19pm: Left message for James Farner requesting disposal information. <br /> Emailed a Notice to Abate to Alan.maas@bnsf.com and james.farner@bnsf.com <br /> 03/09/2017 Completed prop 65 notification.
