DRAFT <br /> Site Monitoring and Documentation <br /> A field log of work procedures, including the documentation of the type of equipment <br /> used,field methods, and procedures implemented will be maintained by DEI personnel. <br /> Sample Documentation,Custody,Shipment and Documentation <br /> All samples will be recorded on DEI's Chain-of-Custody records. The labels will include <br /> the project name, sample identification number, sampling depth, the time and date of <br /> sampling, and the sample preservation method. <br /> Sample Handling and Chain-of-Custody <br /> Upon sample recovery and labeling, the sample will be placed in an insulated ice chest <br /> containing "blue ice" and/or ice. The ice chest containing the samples also will be <br /> accompanied with chain-of-custody (COC) documentation. The COC records will contain <br /> the following information: <br /> • Site name <br /> • Site address <br /> • DEI's project number and task number <br /> • Laboratory's name <br /> • Required turn-around time <br /> • Name of sampler <br /> • Signature of sampler <br /> • Sample identification <br /> • Date of sample's collection <br /> • Time of sample's collection <br /> • Matrix of sample collected <br /> • Required analysis <br /> The sampler will visually inspect the samples to ensure that the sample container integrity <br /> is maintained with no apparent leakage or incorrect packaging. DEI personnel will <br /> complete the COC and sign it prior to sample transfer. Whenever the samples are <br /> transferred from one party to another, both parties sign the COC and record the date and <br /> time of transfer.
