June 4, 1991 <br /> File No. 1525-69 <br /> Page 2 of 3 <br /> Property Location and Authorization <br /> The property, on which the proposed monitoring well will be installed, is located at the <br /> southeast corner of East-Arch and Kingsley Roads in San Joaquin County just south of the <br /> city limits of Stockton. The stress address of the property is 3568 East Arch Road and the <br /> property is identified;as A.P.N. 181-120-01 in County Assessor's book. The current record <br /> owners of the subject property are Obed W. Hill and Mildred 0. Hill. The property owners <br /> had signed a letter of agency with BCEI authorizing environmental assessment at their <br /> property. Since monitoring well installation is part of the assessment, authorization for <br /> entry and well installation is;covered under this,letter�of agency. A_copy_of_the consent ,-- <br />- <br /> letter is attached. <br /> Well Installation <br /> The proposed monitoring well will be drilled and installed by State licensed water well <br /> drilling contractor Bay Area Exploration (BAE) of Suisun City. The drilling contractor's <br /> license number is C57-522125. Balbi & Chang Associates were given the power of agency <br /> by BAE for applying the well permit. A copy of.the letter of agency is also attached. <br /> The proposed boring/well will be drilled with 3.75" I.D. hollow stem augers. The well <br /> casing will be 2" I.D. PVC schedule 40 slotted and blank pipes. The annular space between <br /> the casing and bore hole will be.filled with sand pack, bentonite seal, and cement grout. <br /> The well head will be protected by locking cap and water tight vault. <br /> Well Maintenance and Abandonment <br /> The expected service life for the well is less than one year. ,There is no other scheduled ; <br /> testing other than the initial sampling and testing; Also, we do not anticipate any periodic ! <br /> maintenance will be required during the service,life of the well. The security of the well <br /> will be provided by the property owner, who will regulate the access to his property. At <br /> the end of well service life, the well will be abandoned in accordance with State Water Well <br /> Standards Part III Sections 20 through 23. Abandonment work will be contracted out by <br /> the property owners or their agent at that time. <br />
