Distance to production wells for municipal, domestic, agriculture, <br /> industry and other uses within 2000 feet of the site, ". <br /> 2. Site maps, to scale, of area impacted showing locations of former <br /> and existing tank systems, elevation contours, gradients, and <br /> nearby surface waters, buildings, streets, and subsurface utilities, <br /> 3. High and low ground water levels (below ground surface), <br /> 4. Tabulated results of all sampling and analyses, <br /> i <br /> 5. Vertical and lateral concentration contours of contaminants found <br /> initially, and those remaining in soil and ground water, both on <br /> and off site, <br /> i <br /> 6. Mass balance calculation of the substance treated versus that j <br /> remaining, <br /> 7. Technology used to clean the site; if Best Available Technology <br /> (BAT) was not used, explain why, <br /> 8. Zone of influence calculated for the subsurface remediation system <br /> and the zone of capture attained for the soil and ground water <br /> remediation system, <br /> 9. Reasons why "background" was/is unattainable using BAT, and <br /> 10.
