B St. <br /> Stockton, CA <br /> San Joaquin County Public Health Services, Environmental Health'Division (PHS/EHD) has reviewed the <br /> "Preliminary site Investigation Report" submitted by Alton Geoscience dated July 9, 1992 for the above <br /> referenced site and has the following comments.. <br /> A second round of groundwater sampling and additional groundwater level measuring should be <br /> conducted to confirm the first groundwater sample results and provide more information on the site <br /> specific gradient (conflicting gradients exist between this site and 1603 South B Street). <br /> Additional_montoring wells-and/or_bonngs should-be instalhed'to;define the-TateralTand vemcal.extent of <br /> soil'and groundwatencontamtnation.-A.workplan'for this'aspect'of:investigationahould_besubmittedao3,: <br /> th s_o.ffice:.by.November-.30,-1992: A permit application and $89 fee for installation of additonal <br /> monitoring wells should be submitted to PHS/EHD with the workplan. <br /> Pursuant to Section 2652(d) of the Underground Storage Tank Regulations (California Code of <br /> Regulations, Title 23, Chapter 16) a quarterly monitoring report shall be submitted to Public Health <br /> Services, Environmental Health Division and Central Valley Regional Water Quality Control Board <br /> (CVRWQCB) until the investigation and cleanup of the site is deemed adequate. The report is to show <br /> the: <br /> -status of investigation and:cleanup activities including,the results of all investigations <br /> completed to date; <br /> -proposed continuing or next phase of investigation; <br /> -method of cleanup proposed or implemented, schedules for the completion of the <br /> investigation of the site and remediation; <br /> -method and location of disposal of the released hazardous substance and any <br /> contaminated soil, groundwater or surface water; <br /> -manifest required for transport of hazardous substances; <br /> -monitoring well data including groundwater elevation, pH, conductivity, gradient; <br /> -analytical results of groundwater or soil sampling; and <br /> -site map showing the "zero line." of contamination, if known, and changes in the above <br /> measurements over the last quarter., - <br /> A Division of San Joaquin County Health Care Services '� <br />
