(Geocon), on your behalf for the above-referenced <br /> site. <br /> In Groundwater Monitor Well Destruction Workplan, Geocon proposed destruction of a total of 22 <br /> wells. Geocon will destroy these wells by over drilling groundwater monitoring wells MW-1 <br /> through MW-6, MW-10 thorough MW-19, vapor extraction wells VW-1, VW-2, air sparge AS-1 and <br /> observation well OW-1 through OW-3 using 8" or 11" diameter augers. In the work plan, your <br /> consulting firm, Geocon has proposed to submit a Well Destruction Report after all wells have <br /> been destroyed and data summarizing the field activities. In addition to the data, include a signed <br /> manifest of all water/soil waste related to the destruction of all the wells at this site. <br /> Geocon proposed to use neat portland cement as the sealing material for the wells. All water <br /> used for the purpose of sealing a well must be potable water and not water that has been <br /> displaced from a well that is to be destroyed. With these conditions, the EHD approves the work <br /> plan as submitted. Please submit a completed Master File Record form, a Well and Boring Permit <br /> Application form, and a permit/inspection fee of $3,125.00. Notify the EHD for inspection of all <br /> destruction of wells at least two weeks prior to destruction activities.
