Submit in writing clarification of exactly how many on-site wells will be used in the <br /> test, how monitoring of the tracer gas during the test will be done and specifically <br /> what `observation points' PSI is referring to. <br /> 2. Submit a copy of the referenced attachments. <br /> 3. Submit a site specific Health and Safety Plan. <br /> 4. Notify PHS/EHD forty-eight hours prior to initiation of fieldwork. <br /> 5.
