Project No S8100-06-1113 <br /> April 19, 1996 <br /> 2 0 SCOPE OF SERVICES <br />' The scope of services requested by Caltrans included the installation of four perimeter groundwater <br /> monitoring wells in an to attempt to define the lateral limits of dissolved TPHg in groundwater, the <br /> advancement of one soil boring within the former pump dispenser island to evaluate source soil <br /> ' hydrocarbon impacts, the installation of one vapor extraction well adjacent to the former tank cavity, <br /> and the collection of soil and groundwater samples for analytical testing <br /> Outlined below is a summary of the scope of services to be performed by Geocon under Task Order <br /> 10-5F 1000-14 <br /> 21 Pre-Field Activities <br /> a A pre-work site visit was conducted on December 21, 1995 and attended by Mr Ian Moorhead of <br /> ' Geocon and the Caltrans contract manager, Mr Saiyed All,to survey the project area and review <br /> the Task Order The Site Visit Checklist and Completion Schedule was subsequently discussed <br /> and signed by the Geocon project manager and the Caltrans contract manager <br /> a Obtained well permits from the San Joaquin County Environmental Health Division (SJCEHD) <br /> A copy of the well permit is presented as Appendix A <br /> o Contacted the local public utilities via Underground Service Alert (USA Ticket No 18583) on <br /> January 23, 1996, to advise subscribing utility companies of the soil boring activities and to <br /> attempt to delineate subsurface public utilities and conduits in proximity to the boring/well <br /> locations <br /> 1 o Prepared a Workplan dated January 8, 1996, which described the requested scope of services, <br /> quality assurance/quality control, sampling and laboratory procedures, and project schedule <br /> o Pre a site Prepared specific Health and Safety Plan dated December 28, 1995 to provide guidelines on <br /> p p <br /> the use of personal protective equipment (PPE) and the health and safety procedures to be <br /> implemented during the field activities <br /> o Retained the services of a Caltrans approved and C-57 licensed subcontractor, V&W Drilling, to <br /> perform the drilling activities utilizing hollow stem auger drilling techniques <br /> o Retained the services of a Caltrans approved and California-certified laboratory, Sparger <br /> Technology Incorporated, to perform the analytical testing services <br /> ' 3 <br />
