All copies of each receipt shall contain all of the following information: <br />- The name, address, identification number, contact person, and telephone number of the generator, and the <br />signature of the generator or the generator's representative; <br />- The date of the shipment; <br />- The manifest number; <br />- The volume or quantity of each waste stream received, its California and RCRA waste codes, the waste stream <br />type listed in subdivision (c), and its proper shipping description, including the hazardous class and United <br />Nations/North America (UN/NA) identification number, if applicable; <br />- The name, address, and identification number of the authorized facility to which the hazardous waste will be <br />transported; <br />- The transporter's name, address, and identification number; <br />- The driver's signature; <br />- A statement, signed by the generator, certifying that the generator has established a program to reduce the <br />volume or quantity and toxicity of the hazardous waste to the degree, as determined by the generator, to be <br />economically practicable. <br />Ensure that all future receipts for hazardous waste shipments using a consolidated manifesting procedure contain all <br />required information. <br />This is a minor violation. <br />605 CCR 66262.34(f) Failed to completely label containers or tanks of hazardous waste. <br />A blue steel container of used oil was observed in the front shop labeled as "used floor sweep" with the address for the <br />other facility (1016 S. Wilson Way). A 55 gallon black steel drum of floor sweep was observed with no hazardous <br />property or physical state marked. All hazardous waste containers shall be marked with the following information: <br />- the words "Hazardous Waste" <br />- name and address of generator <br />- hazardous properties (ignitable, corrosive, reactive, toxic) <br />- physical state (liquid, solid) <br />- composition (contents) <br />- accumulation start date <br />Immediately label these containers with accurate information and ensure that all hazardous waste containers are <br />marked with all the required information. <br />This is a repeat violation, Class II. <br />I Overall Inspection Comments: <br />Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br />documenting the corrective actions that have been or will be taken for each violation, and any supporting <br />paperwork, by October 18, 2015. <br />Received by Inspector: Phone: Date: <br />(initial): :4 I STACY RIVERA, Senior REHS 0 (209) 468-3440 09/18/2015 <br />Page 5 of 6 <br />
