Banner Avenue SWRCB-CUF#: 018534 <br /> Lodi, CA. 95242 <br /> WORKPLAN: Work Plan for Monitoring Well Installations <br /> DATED: April 24, 2006 <br /> PREPARED BY: Apex Envirotech Inc.—Michael Sgourakis, R.G. <br /> STATUS: APPROVED with conditions noted below. <br /> ADEQUATE AND NECESSARY ELEMENTS OF THE WORKPLAN rHealth & Safety Code (H&S) <br /> Section 25296.10(c)(3)1: <br /> • Install seven (7) 2" PVC monitoring wells (MW-3C, MW-7A&713, MW-8A&813, and <br /> MW-96 &9C) on and offsite at shallow and deeper intervals (A, B and C zones) to <br /> continue the investigation for the lateral and vertical extents of the MtBE ground <br /> water plume that is endangering two community service/drinking water wells and, <br /> • Collect and analyze selected soil and ground water samples for TPHg, BTEX, seven <br /> oxygenates and, <br /> • Submit a report of findings to EHD. <br /> COMMENTS AND CONDITIONS: <br /> • Submit one Well Permit Application Form and a$368 fee for each parcel where a <br /> well is proposed and a signed access agreements for each private parcel. <br /> • The ground water MtBE plume appears to be descending around MW-4A/413 area. <br /> With MtBE noted in ground water samples from the deeper well, MW-413, a "C" <br /> zone monitoring well is required in this area. EHD directs you to install MW-4C in <br /> this area during this phase of work. <br /> • Elevated levels of MtBE have been detected in ground water samples from MW-3A <br /> and 313. The lateral and vertical investigation to the North, Northwest, and <br /> Northeast of this area has not been addressed in this proposal and a delineation <br /> investigation is required.
