Banner Avenue <br /> Lodi, CA. 95242 <br /> WORKPLAN: Work Plan for Monitoring Well Installation and Interim <br /> Remedial Action <br /> DATED: October 17, 2005 <br /> PREPARED BY: Apex Envirotech Inc. — Michael Sgourakis, R.G. <br /> STATUS: APPROVED with conditions noted below. <br /> ADEQUATE AND NECESSARY ELEMENTS OF THE WORKPLAN [Health & Safety <br /> Code (H&S) Section 25296.10(c)(3)1: <br /> • Install five (5) onsite 6-inch PVC extraction wells for interim ground water <br /> extraction to control current MtBE offsite migration and potential impact to <br /> Community Service Well approximately 700' down gradient and, <br /> • Extract and treat contaminated ground water and install piping for <br /> discharge to private agricultural land under appropriate permits, access <br /> agreement, and requirements of CVRWQCB General Order 5-00-119 and, <br /> • Install four (4) offsite, down-gradient 2-inch monitoring wells in County <br /> right of way at "A-Zone" (depth 5'- 25' below ground surface-bgs), "B - <br /> Zone" (40'-50' bgs), and "C-Zones (70'-75' bgs) for lateral and vertical <br /> investigation required by EHD and, <br /> • Collect and analyze at least three soil samples for TPHg, BTEX, seven <br /> oxygenates and, <br /> • Develop, survey, and add new monitoring wells to existing quarterly <br /> monitoring, sampling, and reporting requirements and, <br /> • Submit a report of findings to EHD. <br /> COMMENTS AND CONDITIONS: <br /> • EHD re-emphasizes it's concern that the priority for this site is ground <br /> water plume migration control. All efforts are to be aimed at maintaining a <br /> ground water extraction rate that exceeds the rate at which the <br /> contaminated ground water is flowing off/away from the site. Treatment of <br /> the purge water is secondary to ensuring that the migrating mass in the <br /> ground water does NOT reach the community drinking water wells. <br /> • EHD hereby directs you to sample MW-4A and MW-4B for MtBE, TBA and <br /> TPHg on a weekly basis for at least one month and report those results to <br /> EHD before each subsequent sampling event. <br />
