• • <br /> Remediation Well Installation Work Plan New West Flag City Facility <br /> ' August 7 2008 6437 W Banner Street, Lodi, California <br /> ' Granular Activated Carbon (VGAC) vessels and three 1,000-1b Liquid Granular Activated <br /> Carbon(LGAC) vessels were installed at the Site and began operation on September 9, 2006. <br /> ' For the purpose of further delineating the lateral and vertical extent of the contaminant plume, as <br /> well as to install sentry wells near CSA-31-1 and CSA-31-2, monitoring wells MW-3 C, MW-4C, <br /> ' MW-7A, MW-713, MW-8A, MW-8B, MW-913, MW-9C and MW-91) were installed by APEX in <br /> July 2006. <br /> ' On May 1, 2007, the New West facility was issued Cleanup and Abatement Order No. R5-2007- <br /> 0709. On May 8, 2007, Closure Solutions, Inc. became the Environmental Consultant of <br /> ' Record for the Site. From May 24, through May 29, 2007, Closure Solutions modified the <br /> existing groundwater extraction system treatment train to remove the air stripper and replace the <br /> ' VGAC and LGAC vessels with two 2,000-1b and one 1,000-1b LGAC vessels. <br /> On November 15, 2007, Closure Solutions submitted a Preliminary Investigation and Evaluation <br /> ' Report (PIER) for the installation of 8 newly installed groundwater monitoring wells (MW-IOA <br /> through MW-IOD, MW-IIB, MW-12A, MW-13A and MW-13B) and collection of four soil <br /> ' vapor samples (SV-1 through SV-4). <br /> On November 30, 2007, Closure Solutions oversaw the reinstallation of monitoring well <br /> MW-IOD due to an obstruction in the well at approximately 15 feet below ground surface (bgs). <br /> ' On November 30, 2007, Closure Solutions submitted a Risk Assessment Report evaluating the <br /> human health and environmental risk associated with impact at the Site. Modeling indicated that <br /> ' the concentrations of contaminant vapors both currently present in soil beneath the Site and <br /> vapors emanating from underlying contaminated groundwater will not create a condition of <br /> significant health risk. <br /> ' Under the direction of the Regional Board, on January 29, 2008, Closure Solutions submitted a <br /> Preliminary Assessment Report (PAR). In accordance with the Tri-Regional Guidelines, this <br /> report included: <br /> ' • The depth and extent of free product found, including an estimate of volume removed <br /> and volume remaining. <br /> ' • Figures delineating lateral and vertical extent of soil contamination, groundwater <br /> degradation plume(s), and vapor plumes as appropriate. <br /> ' Page 3 of 9 <br />
