The detections were just above reporting limits. TAME was detected at wells
MW-3A, EW-3, MW-313, and MW-413 at low concentrations.
' On February 15, 2006, Apex submitted a report titled Groundwater Monitoring Well and Extraction Well
Installations Results and Fourth Quarter 2005 Groundwater Monitoring Report that summarized the
installation and sampling of monitoring wells MW-5A, MW-513, MW-613, and MW-6C and extraction
' wells EW-1 through EW-4 along with the fourth quarter 2005 groundwater monitoring and sampling
results.
' In January 2006 the SJCEHD issued a letter requesting that a aquifer pump test be performed, and that
sampling be performed in accordance with National Pollutant Discharge Elimination System (NPDES)
' requirements. SJCEHD also required the evaluation of remedial alternatives for the Site.
In January 2006,Apex performed an enhanced tracer test on the UST system to identify the source of the
' petroleum hydrocarbon release. The tracer test was unable to identify the source of the release — no
deficiencies of the fueling system were detected during the enhanced tracer test. The source of the release
was considered to be undetermined.
On March 22, 2006, Apex prepared and submitted a groundwater pumping test result report that
summarized the results of the groundwater pumping test conducted at the Site in February 2006. Apex
concluded the data results for the pumping test were inconclusive because the aquifer was not stressed
enough during the tests.
' In March 2006, Apex, Mr. Moore, SJCEHD, and the RWQCB-CVR met again to discuss the current Site
status, environmental concerns, and future environmental work.
' On March 30, 2006, Apex prepared and submitted an Interim Remediation Plan Letter including a
proposal to submit a Work Plan for additional wells at the Site.
' From April 4, 2006 to August 23, 2006, weekly batch extraction was performed at the Site.
Approximately 103,150 gallons were removed from Site for treatment and disposal. The batch extraction
' removed approximately 38.73 pounds of MTBE.
On April 20, 2006, the RWQCB-CVR issued a letter stating the National Pollution Discharge Elimination
' System (NPDES) Permit Application was incomplete and requested more information in order to process
the application.
' In April 2006, Apex submitted a work plan proposing the installation of seven on-Site and off-Site
monitoring wells to further define the vertical and lateral extent of the MTBE plume. The work plan was
subsequently approved by the SJCEHD.
' On May 24, 2006, Apex submitted a letter titled Resample of Wells MW-613 and MW-6C that included
' the results from wells MW-613 and MW-6C that were resampled on May 17, 2006 to confirm first quarter
2006 event detections.
' Page 3 of 7 Closure Solutions, Inc.
