Stantec <br /> July 10, 2013 <br /> Page 2 of 3 <br /> RE: MW-2 Well Destruction Work Plan <br /> Olin Chlor Alkali Facility, 26700 South Banta Road, Tracy, California <br /> The Site-specific Health and Safety Plan (HASP) will be updated for use by personnel implementing <br /> the Work Plan. The HASP will address the proposed activities. A copy of the HASP will be <br /> available on-Site at all times. The subcontractor(s) performing the field activities will be provided a <br /> copy of the HASP prior to initiating field activities. The on-Site subcontractors will also be required <br /> to maintain their own HASP. <br /> Task 2 — Monitoring Well Abandonment <br /> Well MW-2 is a 4-inch-diameter polyvinyl chloride (PVC) monitoring well with a total depth of 25 feet <br /> below ground surface (bgs). A copy of the well construction log is included as an attachment. Well <br /> MW-2 will be destroyed by over-drilling using hollow stem augers. Well casing and installation <br /> material will be removed by advancing augers to the total depth of the well. Augers will be of equal <br /> or greater outside-diameter than those used to install the well to ensure the complete removal of <br /> the well material. The resulting borehole will be backfilled with cement-grout slurry through a tremie <br /> pipe. The surface completion will be removed and the ground surface will be repaired to match <br /> surrounding conditions to the extent possible <br /> Well abandonment will be performed by Gregg Drilling and Testing of Martinez, California (C-57 <br /> License #485165). Stantec will provide an on-Site geologist to oversee Gregg Drilling and <br /> document the well destruction activities. <br /> Well abandonment activities will follow applicable guidelines established in the California <br /> Department of Water Resources (CDWR) Bulletin 74-81, (1981), and CDWR Bulletin 74-90 (1991). <br /> Task 3 — Investigation Derived Waste Disposal <br /> Wastes generated during the over-drilling of the monitoring wells will be stored in 55-gallons drums. <br /> The drums will remain on-Site pending receipt of analytical data from the laboratory for profiling <br /> purposes. The analytical results will be used for waste profiling and disposal evaluation. <br /> Task 4— Reporting <br /> Documentation of the well abandonment will be submitted as part of the 2013 Semi-Annual Report. <br /> Documentation will include a brief written summary, waste profile results, permits, and other <br /> pertinent information. <br /> State of California Well Completion Reports (DWR Form 188)will be submitted to the California <br /> Department of Water Resources. <br /> SCHEDULE <br /> Stantec proposes to abandon well MW-2 in July 2013.
