Geosyntec consultants Geosyntec HSE Procedures referenced herein are available on Geosyntec's HSE SharePoint site TASK HAZARD ANALYSIS and should be consulted,as appropriate,to ensure requirements are met.This THA has been (ver.4.0,9-8-2023) prepared per"HSE STD-105.Proiect Safety Management Handbook" Part A — PROJECT/TASK INFORMATION Project/Site Name: Olin Chlor Alkali Facility Project Number/Org: WR3313A Site Address: 26700 South Banta Road,Tracy, CA Task&Worksite Geosyntec will oversee the decommissioning of one 25-ft deep,2-in diameter groundwater monitoring well(MW-6 in Description: Work Plan)by pressure grouting and over-drilling in accordance with SJCEHD specifications.Work will be conducted by a C-57 licensed drilling subcontractor using a"Rhino"hollow-stem auger/direct-push combo rig. • MW-6 will be tagged to verify total well depth. • The well box will be removed,and the well will be over-drilled to a depth of approximately 25 feet using an HSA drill rig.The well casing,filter pack,and annual seal will be removed. Following over-drilling,Portland cement mixed with 3-5%bentonite gel(cement-bentonite grout)will be used to backfill the boring to approximately 1 foot below the ground surface.The upper 1 foot of each borehole will be backfilled with native material to match the surrounding ground surface. • MW-6 is located within Union Pacific Railroad right of way.An access agreement has been obtained. Geosyntec will hire a UPRR-approved flagging service to watch out for and direct train traffic during well destruction.Geosyntec staff will receive Union Pacific Roadway Worker Protection training(available on LINK portal)before start of the work. • In addition, previous boring locations GGW-1 and GGW-2, located in an orchard located to the east,will be surveyed. • Well destruction IDW will be temporarily contained on site in 55-gallon drums by the drilling subcontractor before being disposed offsite by a waste disposal subcontractor. • A porta-porty will be provided for the day of work.
