0 3 • • <br /> Julyuly 31, <br /> i,20 20 03 <br /> Page 2 of 3 <br /> IMMEDIATELY FOLLOWING COMPLETION OF WORK PERMITTED HEREIN, <br /> PERMITTEE SHALL FILL OUT AND MAIL NOTICE OF COMPLETION PROVIDED <br /> BY GRANTOR. <br /> Permittee's attention is directed to Section 6, "CONTROL OF MATERIALS", of the State <br /> Standard Specifications Reference to Engineer in the State Standard Specifications <br /> shall include State Representative. <br /> Permittee is directed to Standard Specification section 7-1.11 Preservation of Property, <br /> and Business and Professions Code, Section 8771. Permittee shall physically inspect <br /> the work site and locate survey monuments prior to work commencement. Monuments <br /> shall be referenced or reset in accordance with Business and Professions Code. <br /> A pre job conference with the Permittee, Contractor, and State Representative may be <br /> required prior to start of work. Permittee shall contact the State Representative and <br /> arrange the meeting. This meeting may be waived at the discretion of the State <br /> Representative. <br /> Notwithstanding General Provision #4, your contractor is required to apply for and <br /> obtain an encroachment permit prior to start of work. <br /> Permittee shall, prior to commencement of any work, provide the State Representative <br /> with the name and phone number of the person in responsible charge of the work to be <br /> performed under this permit. <br /> Before commencing work, permittee must obtain written concurrence from the State <br /> Highway Resident Engineer on the scheduling of the proposed work. <br /> All work shall be conducted in such a manner that the excavation, excavated earth, <br /> materials and equipment will not cause any inconvenience to the highway traffic or to <br /> traffic entering the highway from any public or private approach. <br /> Except when necessary, as determined by the State Representative, no installation, <br /> construction equipment or personal vehicles shall operate or park within the traveled <br /> way. <br /> In addition to the attached General Provisions, the following special provisions are also <br /> applicable for installation and monitoring of groundwater monitoring wells: <br /> 1. Unless specifically authorized elsewhere in this permit, survey work shall not be <br /> performed: <br /> A. On holidays or an afternoon proceeding a holiday. <br /> B. In medians. <br /> C. When work requires traffic control on lanes. <br /> 2 <br />
