2.0 WELL DESTRUCTION <br /> 2.1 FIELD DATES <br /> July 20 through 24,2009 and August 6,2009. <br /> 2.2 PERSONNEL PRESENT <br /> Geologist Scott Lewis, Sherry Phillips, and Erin Rienhart-Koylu directed the well <br /> destruction activities under the supervision of California Professional Geologist Joe <br /> Neely. <br /> 2.3 DESTRUCTION METHOD <br /> Wells MW-1 and MW-2 were destroyed by breaching the bottom of the well casing, <br /> backfilling the well casing with Portland Type I/II cement, and then removing the well <br /> casing. This destruction method differs from what we proposed in our work plan, but <br /> this method was approved by the San Joaquin County Environmental Health <br /> Department inspector onsite during destruction activities. The remaining site wells <br /> were destroyed by pressure grouting the well casings using neat Portland Type I/II <br /> cement per San Joaquin County guidelines. Copies of the well destruction permits and <br /> the CalTrans encroachment permit are included in Appendix A. <br /> 241015(4) 2 CONESTOGA-ROVERS&ASSOCIATES <br />
