Therefore, in order to proceed <br /> towards the public participation phase for the closure, you are directed to submit the following: <br /> 1. A property parcel map depicting your site, the extent of your identified hydrocarbon plume, and <br /> all parcels located within 500 feet of your site and/or your defined hydrocarbon plume. <br /> 2. For each parcel, you are to provide a list of the parcel number, the name of the owner for each <br /> parcel, and a mailing address for each owner and all tenants. Also include contact information <br /> for the Caltrans district office responsible for Interstate 5, which is located next to the Site. <br /> 3. A public notification document (Fact Sheet) for our review. The Fact Sheet should contain a very <br /> brief site history, discussing investigative and remedial (if applicable) activities conducted at the <br /> site, current site conditions, and your findings that the residual constituents mass remaining in <br /> soil and groundwater beneath your site does not pose a threat the human health or waters of the <br /> state. The Fact sheet must also state your intent to obtain site closure from the Regional Board, <br /> and include a method by which any interested individuals may obtain additional information <br /> regarding the site or make comments regarding the possible closure of the site. You are to list <br /> the Regional Board as the contact for questions and comments, using my contact information in <br /> this letter: <br /> Once I receive the requested information, I will continue the closure review process for your site by <br /> providing comments and directing the start of the 45-day public participation period.
