Ben Holt Drive ROM 00486 <br /> Stockton, CA. 95219 SW RCB-CUF#: 015906 <br /> In a letter dated August 29, 2006, the San Joaquin County Environmental Health Department <br /> (EHD) informed the Executive Officer of the California Regional Water Quality Control Board, <br /> Central Valley Region (CVRWQCB), that the EHD had determined that the site investigation and <br /> remedial action of contamination resulting from an unauthorized release from the underground <br /> storage tanks formerly located at the above referenced site had been completed as is required in <br /> our Local Oversight Program contract with the State Water Resources Control Board. In a letter <br /> dated December 15, 2006, the CVRWQCB Executive Officer did not concur with the EHD's <br /> determination. This letter is to inform you and by copy of it to the CVRWQCB that the EHD is <br /> transferring the site to the CVRWQCB for further oversight. <br /> Please be advised that the San Joaquin County Ordinance Code requires well permits be <br /> obtained from the EHD whenever any well or boring is constructed or destroyed, including <br /> boreholes for environmental investigation, and that the fieldwork be inspected. The EHD must <br /> charge for the staff time spent permitting and inspecting these well/borings. The current hourly <br /> rate charged by the EHD is $95. To process future well/boring permit applications needed to <br /> conduct work required by CVRWQCB, an account must be established with the EHD. Enclosed <br /> is a Master File Record (MFR)for you to complete and return to the EHD with a payment of$285 <br /> (minimum three hours of work). This payment will be credited to your account code for this site <br /> and is required before the next well/boring permit can be issued.
