COMMENTS AND CONDITIONS: <br /> 1) The Environmental Health- Department (EHD) requires six well destruction, permit <br /> applications. @ $375.00 each to be submitted. Individual application sites include; Arco <br /> site, Chevron site,- City of Stockton parcel and Stockton easements, Cal-Trans <br /> -easement, and two private parcels. Access and easement agreements will need to be <br /> obtained from all designated parcel owners. <br /> 2) A Master File Record must be completed and submitted with the EHD.permits. <br /> 3) A Well Destruction Report shall be submitted within 90 days of the completion of the <br /> well destruction activities including the removal of the waste materials and all manifests <br /> associated with the removal. <br /> 4) A total of forty-four wells are to be destroyed at $125:00 per well. EHD permits must be <br /> approved prior to commencing well destruction work.
