I <br /> COMMENTS AND CONDITIONS: - <br /> 1) The Environmental Health Department (EHD) requires six- well destruction permit <br /> applications @ $375.00 each to be submitted. Individual application sites include; Arco <br /> site, Chevron site, City of Stockton parcel and Stockton easements, Cal-Trans <br /> ccess and easement a reements <br /> will need to be <br /> easement, and two private parcels. A - 9 <br /> obtained from all designated parcel-owners. <br /> 2) A Master File Record must be completed and submitted with the EHD permits. <br /> 3) A Well Destruction Report shall be submitted within 90 days of the completion of the. <br /> well destruction activities including the removal of thewaste-materials and all manifests <br /> associated with the removal.. <br /> 4) -.A total of forty-four wells are to be destroyed at $125.00 per well.l-EHD permits must be <br /> approved prior to commencing well destruction work.
