Chuck Carmel Page 2 Please submit PHS/EHD permit applications for the installation of the off site monitoring well and for the soil borings by March 16, 1993. Additional soil boring's locations may be indicated on the site map accompanng the permit applications. PHS/EHD, in correspondence dated May 27, 1992, approved the "Addendum to the draft Final Remediation Plan";dated April 29, 1992 and prepared by BC,which proposed the installation and operation',of soil vapor extraction and treatment system, to remediate soil contamination. PHS/EHD requested!in correspondence dated May 27, 1992, August 28, 1992, and November 23, 1992, that application be made to the San Joaquin Valley Unified Air Pollution Control District (SJVUAPCD) to obtain an'"Authority to Construct permit. PHS/EHD has been informed that as of February 19, 1993 no application is on file with the SJVUAPCD. Please submit by March 16, 1993 an application to the SJVUAPCD for an'Authority to Construct permit. PHS/EHD, in correpondence dated November 23, 1992, approved the letter report dated September 23, 1992 and prepared by BC,which amended the number of vapor extraction wells to be installed from five to twelve wells. Please submit a work plan to install these vapor extraction wells.by May 17, 1993. As per your February 11, 1993 telephone conversation a meeting has been tentatively'scheduled for March 16, 1993 at 9:00 a.m. to discuss ARCO's sites in San Joaquin County. The meeting is to be held at the office`.of PHS/EHD which is located at 445 North San Joaquin Street, Stockton CA.
