year, or upon change (permanent or temporary) to the <br /> premise or volume of the wastewater discharge, or <br /> deviation from the terms and conditions under which this <br /> Permit is granted. <br /> 12 . Permittee shall maintain all pretreatment records for at <br /> least three years All accidental spills or upsets must <br /> be reported to this Department immediately. This shall <br /> be accomplished by telephone, and shall be followed by <br /> written report within five working days by _Permittee to <br /> correct the situation and to prevent its' recurrence. <br /> Failure to report a spill or upset will bei grounds for <br /> civil or criminal penalties , depending upon <br /> circumstances . <br /> 13 . For all hazardous wastes (as defined in 40CFR Part 261) <br /> stored or hauled from site, Permittee must meet all <br /> applicable regulations (40CFR Parts 260 through 271) , <br /> and any applicable State regulations promulgated <br /> subsequent to the 'Federal law. <br /> 14. Permittee shall prepare and submit a spill prevention <br /> plan within 30 days from the effective date of this <br /> Permit to assure that liquid wastes are contained onsite <br /> at all times . <br /> 15 . The discharge of excess amounts of: pollutants <br /> specifically limited and deviations of pH may be subject <br /> to fines for each violation. <br /> 16 . Achieving Permit Conditions of specific pollutant <br /> concentration limits by dilution is expressly <br /> prohibited. Any evidence of this , either willfully or <br /> through negligence, will result in Permit ,Limits being <br /> redefined in terms of maximum effluent loadings . <br /> Additionally, penalties of a maximum of $6 ,000 per day <br /> may be assessed (City Code 7-091. 12) . <br /> LDW:tw <br /> 3/89 <br /> y <br /> I <br />
