Three (3) prints of the Project Site Map indicating <br /> existing and proposed tanks and piping. <br /> 2 . Three (3) copies of the application for Permit for <br /> Underground Tank Closure. Please note that <br /> contractor information cannot be provided at this <br /> time, but will be provided to you at the earliest <br /> possible date. <br /> 3 . Three (3) copies of a completed Authorization to <br /> Release Analytical Data form. <br /> 4 . A completed Underground Tank Program Fee worksheet. <br /> 5. A check in the amount of $480. 00 for closure and <br /> plan check fees. <br /> 6 . Closure Plan Information Sheet indicating procedures <br /> to be followed during the removal of the underground <br /> tanks and piping. <br /> 7 . An incomplete Underground Tank Disposition Tracking <br /> Record. <br /> Please note that we do not have a contractor for this <br /> project as of this date. The project (both the removal and <br /> installation of tanks at this site) will be going out to bid within <br /> the next two (2) weeks.
