Four (4) complete sets of installation plans. <br /> 2. A completed Underground Storage Tank Program Fee Worksheet. <br /> 3. A check in the amount of $454.00 for plan check and tank fees. <br /> 4. Three (3) copies each of completed Form A and four (4) completed <br /> Form B's for the new tank installation. <br /> 5. A copy of both the Response Plan and the Monitoring Plan for this <br /> facility. <br /> 6. Copies of cut sheets for the following items: <br /> A. The Xerxes double-wall tanks. <br /> B. The OPW233-BM-4422 Extractor Fitting. <br /> C. The OPW53-VML Float Vent Valve. <br /> D. The Red Jacket Submersible Pump. <br /> E. The OPW104-AOW Observation Manhole. <br /> F. The OPW 634-TT-7085 Fill Cap and OPW 633-T-8075 Adapter. <br /> G. The OPW 1711-T-7085 Vapor Recovery Cap and OPW 1611-AV-1620 <br /> Vapor Recovery Adaptor. <br /> H.
