NAME- ARCO PRODUCTS COMPANY PERMIT f 1094-6SV 0914 DATE December 20, 1994 i Permittee shall, prior to commencement of any work, provide the State representative with the name and phone number of the person in responsible charge of the work to be performed under this permit In accordance with General Provision No 6, TWO WORKING DAYS PRIOR to the start of this work, permittee shall notify the state representative BILL LsMASCUS, P.O BOX 2048, Stockton CA, 95201, (201c- 948-3656 All work under this permit must meet with the approval of the state representative Failure on the permittee's part to comply with any provision will be cause for revocation of this permit. IMMEDIATELY FOLLOWING COMPLETION OF WORK PERMITTED HEREIN, PERMITTEE SHALL, FILL OUT AND MALL NOTICE OF COMPLETION PROVIDED BY GRANTOR. Permittee's attention is directed to Section 6, "CONTROL OF MATERIALS", of the State Standard Specifications. Reference to Engineer in the State Standard Specifications shall include State Representative Permittee is directed to Standard Specification Section 7-1 11 Preservation of Property, and Business and Professions Code, Section 8771 Permittee shall physically inspect the work site and Iocate survey monuments prior to work commencement Monuments shall be referenced or reset in accordance with Business and Professions Code This permit is not valid until permittee has obtained permission from adjacent property owners, Irrigation Districts, Cities, Counties or other interested parties to perform the proposed work . In addition to the attached General Provisions, form DM-M-P-202B, the following special provisions are alsc applicable: 1 Unless specifically authorized elsewhere in this permit, survey work shall not be performed: a. On holidays or an afternoon preceding a holiday. b. In medians. c. When work requires traffic control on lanes. 2. Permittee shall furnish, place and maintain required signs, safety equipment, and traffic control and warning devices in accordance with the Manual of Traffic Controls published by the Department of Transportabon. 3. All personnel shall wear hard hats and orange vests, shirts or jackets as appropriate. 4 Unless specifically authorized in this permit survey, markings within the right-of-way shall be temporary. Paint used for markings shall be water soluble and other markers shall be removed upon completion of survey. 5. Any collected survey data requested by Caltrans shall be furnished to Caltrans without charge. 6. Measurements across traffic lanes shall be made by electronic distance measuring device utilizing nonvisible light or other optical means. . 7. Electromagnetic and radioactive equipment must be operated by certified personnel and shall not interfere with radio communications or be directed toward the traveling public. 8 Vehicles necessary for survey work may be parked in State Highway right-of-way off paved shoulders Auialiary support and employee vehicles shall remain outside the right-of-way.
