Benjamin Holt Dr. <br /> Stockton, CA., 95207 <br /> On November 1, 1996, the thirty (30) day comment period provided to the Central Valley Regional Water <br /> Quality Control Board (CVRWQCB)to dispute the decision by San Joaquin County Public Health <br /> Services, Environmental Health Division (PHS-EHD) to close the investigation of this site ended and no <br /> comments were received. <br /> In order to complete the closure requirements for this site, the monitoring wells still present on site will <br /> need to be addressed. There are two possible options for you to consider. The wells must either be <br /> properly destroyed under a permit issued by PHS-EHD or the wells may remain on active status and you <br /> will be required to monitor and sample them on an established schedule and report the findings to PHS- <br /> EHD for review and evaluation. <br /> Well destruction procedures should be included in a brief workplan that includes materials, a time <br /> schedule, and should be submitted to PHS-EHD prior to January 1, 1997. Please include the$60 permit <br /> fee along with the signed permit application. <br /> If monitoring and sampling of the wells is the best option for you, please contact Michael Infurna so that <br /> the proper forms and other requirements for this program can be discussed with you prior to the next <br /> quarterly sampling period. <br /> Please notify PHS-EHD of your decision as soon as possible so that the proper arrangements can be <br /> made.
