Benjamin Holt Dr.
Stockton, CA., 95207
On November 1, 1996, the thirty (30) day comment period provided to the Central Valley Regional Water
Quality Control Board (CVRWQCB)to dispute the decision by San Joaquin County Public Health
Services, Environmental Health Division (PHS-EHD) to close the investigation of this site ended and no
comments were received.
In order to complete the closure requirements for this site, the monitoring wells still present on site will
need to be addressed. There are two possible options for you to consider. The wells must either be
properly destroyed under a permit issued by PHS-EHD or the wells may remain on active status and you
will be required to monitor and sample them on an established schedule and report the findings to PHS-
EHD for review and evaluation.
Well destruction procedures should be included in a brief workplan that includes materials, a time
schedule, and should be submitted to PHS-EHD prior to January 1, 1997. Please include the$60 permit
fee along with the signed permit application.
If monitoring and sampling of the wells is the best option for you, please contact Michael Infurna so that
the proper forms and other requirements for this program can be discussed with you prior to the next
quarterly sampling period.
Please notify PHS-EHD of your decision as soon as possible so that the proper arrangements can be
made. You may send your workplan or comments to Michael Infurna, Senior REHS, and you may contact
him at(209)468-3454 if you have any questions. PHS-EHD thanks you for your cooperation throughout
this site's investigation and congratulates you on a job well done.
Donna Heran, REHS, Director
Environmental Health Division
Michael Infurna Jr., Senior REHS Margareagorio, RE
LOP/Site Mitigation Unit IV Supervisor
Ml/
c: Bob DeNinno-Southland Corp.
c: Charles Almeida-Delta Environmental Consultants
c: Jack Puller-711 Store Manager
c: James Brathovde-CVRWQCB
A Division of San Joaquin County Health Care Services
