Weldon Church Property Page 4 <br /> 104 West Beverly Place March 3, 2010 <br /> Tracy, California 95376 <br /> The EHD will not approve the proposed RegenOxTM injections until the results from the <br /> aquifer volume test, installation of the proposed monitoring well, and collection of <br /> groundwater from the proposed well and MW-4 to determine background/baseline site <br /> conditions have been submitted to the CRWQCBCVR and the EHD, and the pilot test work <br /> plan has been approved by the CRWQCBCVR as stated in the General Order, Number 2 of <br /> Section A (Conditions of Eligibility). Upon review of the data, if the CRWQCBCVR issues <br /> the permit and the EHD approves the final work plan, the application of RegenOxTM may <br /> begin. <br /> Once the EHD has approved the boring location for performing the preliminary aquifer <br /> volume test, submit a boring permit application and permit/inspection fee of $434 per parcel <br /> to the EHD for the installation of the proposed monitoring well and advancement of the <br /> injection borings and complete the well installation and preliminary aquifer volume test by <br /> June 3, 2010.
