(AGE) has prepared this <br /> Revised Work Plan for Soil and Ground Water Investigation - Addendum for additional soil and <br /> ground water investigation at the referenced site. The Revised Work Plan Addendum is prepared as 7" <br /> directed by the San Joaquin County Public Health Services - Environmental Health Department <br /> (PHS-EHD) in response to Work Plan for Soil and Ground Water Investigation -Addendum, dated <br /> 02 December 1998. <br /> This Revised Work Plan Addendum supplements the original Work Plan for Soil and Ground Water <br /> Investigation, prepared by AGE in December 1995, which was prepared for backfilling the UST <br /> excavation on the property and advancing six soil probe borings to collect soil and ground water <br /> samples (Figure 1). The work was completed during December 1995. Four subsequent work plan <br /> addendums have been prepared and executed to assess and/or monitor petroleum hydrocarbon- <br /> impacted soil and ground water associated with the release of fuel hydrocarbons from the former <br /> underground storage tank (UST) facility. The subsequent addendums, listed below by preparation <br /> date, were prepared for: <br /> • 12 February 1996: installation of four ground water monitoring wells (MW1, MW2, MW3 <br /> and MW4), which were installed during July 1996; <br /> • 08 January 1997: installation of two additional ground water monitoring wells (MW5 and <br /> MW6), completed during April 1997; and installation of six soil vapor probe borings,which <br /> was subsequently not required by the San Joaquin County Public Health Services - <br /> Environmental Health Division (PHS-EHD); <br /> 4005 North Wilson Way,Stockton,California 95205 <br /> Telephone(209)467-1006 FAX(209)467-1118 <br />
