The top two-and-a-half feet or more of the well shall <br /> be drilled out with an auger that is at least one foot larger in diameter than the auger <br /> originally utilized to install the well, and the soil around the cut-off casing removed to <br /> the depth of an additional six inches below the new 'top' of the cut-off casing. The <br /> grout sealant shall spill out of the top of the well casing to form a mushroom cap seal <br /> with at least twelve inches of meat cement grout poured six inches above and six <br /> inches below the top of the cut-off casing. <br /> • All groundwater displaced to the surface during grout installation shall be pumped out of <br /> j <br /> the excavation to a p .. <br /> allow -for EHD ins ection and verification that the well has been <br /> properly destroyed. Contaminated groundwater pumped out of the excavation shall be <br /> containerized for proper disposal. <br /> • This site has wells located off-site in the City of Tracy right-of-way and on nearby <br /> parcels. Please submit a copy of all access agreements and the encroachment permit <br /> issued by the City of Tracy when submitting the EHD Well and Boring Permit <br /> Application form for these wells. <br /> i EHD records indicate the presence of 'nested' wells installed off-site. The EHD has <br /> found destruction results of nested wells are best obtained when augers larger in <br /> diameter than the original augers are used and centered on the deepest casing (which <br /> I is off-center in,the original borehole). The augers need to be sized sufficiently to reach <br /> out and remove all well casings, screens, filter packs and grout sealant installed during <br /> construction. <br /> After the EHD has approved the well destruction scope of work proposal, you may submit an <br /> tl <br /> EHD Well and Boring pp Permit Application form for. each parcel along with a Licensed <br />_ <br /> Contractors' Declaration (LCD), Master File Record (MFR) form, Encroachment Permit, <br /> F access agreements and appropriate fees for each parcel. The permit fee for the destruction of <br /> each well is $130.
