0 <br /> ACCESS AGREEMENT <br /> Well Destruction <br /> 101 West Beverly Place, Tracy, California, 95376 <br /> (APN: 233-152-26) <br /> I, Vicki Myers, the property owner of parcel 233-152-26, do hereby grant permission to <br /> Advanced GeoEnvironmental, Inc. (AGE) on behalf of the responsible party, Weldon <br /> Church, to destroy one (1) dual-completed groundwater monitoring well under San <br /> Joaquin County Environmental Health Department permit at 101 West Beverly Place, <br /> Tracy, California, 95376. The site under investigation is located across the street to the <br /> above referenced property at 104 West Beverly Place, Tracy, California. The proposed <br /> work is being performed under the directive of the San Joaquin County Environmental <br /> Health Department. The scope of work will include the following: <br /> • Destruction of one (1), dual-completed, 2-inch diameter groundwater monitoring <br /> well.Well MW-6A/B,will be drilled-out to one foot deeper(24 feet)then the installed <br /> total depth utilizing a track-mounted limited access drilling rig equipped with <br /> 12.25-inch diameter hollow-stem augers.All original well construction materials will <br /> be removed (including well casing, screen, filter pack, and annular seal) to ensure <br /> that no residual contamination is sealed within the abandoned borehole.
