ACCESS AGREEMENT <br /> Well Destruction <br /> 104 West Beverly Place, Tracy, California, 95376 <br /> (APN: 233-153-13) <br /> I, Juan Manuel Baeza, the property owner of parcel 233-153-13, do hereby grant <br /> permission to Advanced GeoEnvironmental, Inc. (AGE) on behalf of the responsible <br /> party, Weldon Church, to destroy three (3) groundwater monitoring wells and five (5) soil- <br /> vapor wells under San Joaquin County Environmental Health Department permit at 104 <br /> West Beverly Place,Tracy, California,95376(site).The proposed work is being performed <br /> under the directive of the San Joaquin County Environmental Health Department. The <br /> scope of work will include the following: <br /> • Destruction of three(3),2-inch diameter groundwater monitoring wells.Wells MW-1, <br /> MW-2, and MW-8, will be drilled-out to one foot deeper(26 feet) then the installed <br /> total depth utilizing a track-mounted limited access drilling rig equipped with <br /> 8.25-inch diameter hollow-stem augers. All original well construction materials will <br /> be removed (including well casing, screen, filter pack, and annular seal)to ensure <br /> that no residual contamination is sealed within the abandoned boreholes. The <br /> remaining boreholes will be backfilled to surface grade with neat cement and <br /> covered with concrete, asphalt, or native material (soil or grass) depending on the <br /> surrounding surface material. <br /> • Destruction of five (5), 0.75-inch diameter soil-vapor wells (in planter bed). Wells <br /> V-1 through V-5, will be removed from their total depths of 5 feet below surface <br /> utilizing a 3-inch diameter hand auger. The remaining boreholes will be backfilled <br /> to surface grade with neat cement and covered with native material. <br /> • The surface will be returned to the original condition as best as it can. <br /> All expenses for the well destructions performed at 104 West Beverly Place, Tracy, <br /> California shall be paid by the responsible party(Weldon Church), and shall be conducted <br /> under permit and in accordance with local regulations.Reporting of the proposed work at <br /> your property will be forwarded to you the property owner. <br /> We will provide 48 hours notice prior to the initiation of field activities, which shall be <br /> conducted in a manner to minimize disruption to the property.
